Institutional Memory: Why Organizations Repeat the Same Mistakes

A project misses its targets but gets remembered as a success. A recent mistake overshadows months of contributions. An important commitment disappears when a manager changes roles. These situations raise a question about institutional memory: what does an organization actually remember, and what gets lost?

In this episode, business psychologists Dr. D and Dr. K explore how people, records, and shared accounts preserve an organization’s history. They discuss how recency bias shapes performance reviews, how leaders and teams remember the same project differently, and how competing priorities can push earlier commitments out of view. Dr. D shares a project-funding experience that illustrates the consequences of leaving an important decision outside the official budget.

The conversation examines three reasons organizations forget: employee turnover, pressure to address immediate needs, and competing memories of the same events. Dr. D and Dr. K also discuss how organizations distribute credit and accountability, and what employees remember when leaders fail to follow through.

The episode offers practical approaches to preserving knowledge and learning from experience. These include documenting assumptions before making a decision, comparing results with the original business case, conducting a knowledge audit, and keeping a year-round record of contributions and challenges. For people preparing for a performance review, the discussion closes with two actions: document how your responsibilities have changed and bring a complete record of your work.
