One birth center skipped the routine interventions hospitals treated like gospel—and low-risk moms kept delivering just fine. A sharp, surprising look at how radically different childbirth can be.

No Monitors. No IVs. No C-Sections?

Midwives, family docs, OBs—same low-risk births, wildly different C-section rates. Dr. Michele Manting digs into the intervention gap and asks the question nobody in maternity care wants to answer.

Why Are Midwives Cutting Less?

Does breaking the water really speed labor up—or kick off a chain reaction of interventions? Dr. Michele Manting pulls apart one of OB’s most repeated beliefs and asks the question nobody wants to revisit.

The One Pop That Starts the Cascade

One OB-GYN spent four decades questioning the routine—and landed at a 9% C-section rate while the U.S. climbed past 32%. It’s a credibility bombshell with hard-won lessons on trusting birth and cutting the unnecessary drama.

40 Years, 9% C-Sections

"And it got me into trouble."

On this first episode of Off the Cutting Edge, veteran board-certified OB-GYN Dr. Michele Manting looks back on the career-long results of an unusual elective she took during residency.

What was supposed to be a sex therapy rotation with her soon-to-be mentor Sally Schumacher opened her eyes to career-defining moments in terms of practicing women's medicine. The curiosity that led her to request the elective in the first place soon took her to a free-standing birth center in High Point, North Carolina. There she observed first-hand a very different way of caring for women physiologically in childbirth.

She was inspired by Libby Dixon, the certified nurse-midwife in charge of the center. She also surreptitiously left town and spent a few days with Ina May Gaskin at her house at The Farm in Tennessee. To this day she can't let go of the discrepancy between C-section rates between midwives and obstetricians. No matter what they say: low risk is low risk. The definition doesn't change no matter who cares for women in childbirth, but the outcomes do!

Michele describes how one simple research question about the tiniest of labor interventions slam-crashed into obstetric dogma, and how the backlash that followed, and a rethink of what normal labor looks like in person and in the literature, led to her 40-year history of a 9% C-section rate.

Board-certified OB-GYN Dr. Michele Manting has spent 40 years asking why, and kept her C-section rate at 9% while the national rate climbed past 32%. By her estimate, more than half the C-sections done in the US are unnecessary. Off the Cutting Edge shares the stories behind her writing on Substack and her upcoming book, planned for early 2028: the patients, the research and the hard questions behind childbirth and women's health, plus insider tips on how to stay off the cutting edge.
